HPCf staff please use these forms for giving out HPCf (physical) keys. Fill out, sign, and return to the director to obtain a key. Each key has a code stamped on it (eg. HMA, A1, ...), put this code on the form.
Sign the form when returning the key (change, quit, promotion, ...).
